This Provider Search lists medical providers in PersonalCare’s provider network. This data is updated weekly for your convenience. While it is our goal to provide accurate information, our provider network is subject to change without notice.
It is important to know when you enroll in PersonalCare that services are provided through our delivery system, but continued participation of any one doctor, hospital or other provider cannot be guaranteed, even if a member is referred by his/her physician to a provider. We strongly encourage you to call your provider of choice to confirm his/her participation in PersonalCare’s provider network.
Please Note:
- Members who reside in Peoria, Tazewell and Woodford counties can access OSF St. Francis Medical Center and Children’s Hospital of Illinois in Peoria for emergency care only.
- PersonalCare members must have prior authorization for all services at University of Chicago Medical Center and University of Chicago Physician Group.
- The link for University of Iowa Physicians below lists their health care providers that participate in PersonalCare’s network. These providers appear in their own search link because they are not included in the PersonalCare Network link.

PersonalCare’s Freedom PPO and Optimum PPO Level 3 includes Coventry Health Care National Network providers. Please click on the link above to search for Level 3 providers. Coventry National Network providers may not be available in all areas. Providers found in the Level 3 directory may also appear in the PersonalCare Network search tool showing up as Level 1 or Level 2. In such instances, the best payment level applies to your provider visit.
PersonalCare PPO members who live outside of our service area have access to the Coventry National Network. Dependents living outside of the PersonalCare service area and enrolled in the Passport program may access the Coventry National Network for in-network benefits. All PersonalCare members with benefits for bariatric surgery who plan to have weight management surgery and meet the applicable criteria are required to have those services performed by one of PersonalCare’s Bariatric Surgery Centers of Excellence for the services to be covered. Click on the link above to view a listing of these Centers of Excellence. These services also require a prior authorization from PersonalCare and the ordering provider.
